Core Consulting Activities for Process Hazard Analysis

Pre-study data and PSI review – Verification of Process Safety Information (PSI) and preparation of P&IDs, cause-and-effect diagrams, and design data required for PHA.

Process Hazard Analysis using What-If, HAZOP, and/or FMEA – Application of the most suitable method, or a combination of methods, to identify hazards, analyse deviations and failure modes, and prioritise risk reduction and improvement actions.

Risk escalation from HAZOP to LOPA and QRA – Enhance qualitative HAZOP results with semi-quantitative LOPA to evaluate risk, independent protection layers (IPLs), and required risk reduction or SIL targets, and, where needed, extend to full QRA to quantitatively model fire, explosion, and toxic release scenarios and determine individual and societal risk.

Action follow-up and revalidation – Tracking of PHA recommendations, verification of closure, and periodic revalidation in line with regulatory and corporate requirements.
